Volunteer Of The Year Form

The Volunteer of the Year Award was established to recognize individual personal involvement in the development of the cultural arts in Region 8. Nominees should hold leadership positions or share a particular expertise that substantially impacts an arts organization’s contributions to the quality of the cultural life in the parish in which they were domiciled during the past year.

*Past winners are not eligible for nomination if they have won the award within the last two years.

In narrative form, not to exceed 750 words, provide a concise description of the contributions for which the individual is being nominated.  The narrative should address and include, at a minimum, the following:

1)         the accomplishments and/or positions held by nominee in the nominating organization as well as in other cultural or civic organizations with arts related programming.
2)         a description of the nominee’s general volunteer ability.
3)         a description of any long-term impact resulting from the nominee’s volunteer efforts on the future of the nominating organization or the general quality of life of the parish/region.  
4)         a summary statement by the nominee describing his/her attitude toward volunteerism: (this should be in the form of a 1-sentence quote from the nominee as to why he/she feels volunteerism is important to the growth of the arts in northeast Louisiana).

Supplemental materials are not required for the Volunteer of the Year Award however, they can strengthen your nomination. If you choose to include supplemental materials (press clippings, programs, samples of work i.e., slides, videos, photographs, etc.) they must be uploaded at the time of submission of the nomination form.  NO supplemental materials will be accepted after nomination submission.  Any external video links should be copied and pasted to the text box below the click and drag box for Panel Review. ***If you have any issue adding multiples items to the click and drag box, try highlighting them at all at once and dragging them at the same time to the box.
